Social Determinants of Health in Rural Communities Toolkit

The Rural Health Information Hub (RHIhub) created the Social Determinants of Health in Rural Communities Toolkit. This toolkit compiles evidence-based and promising models and resources to support organizations implementing programs to address social determinants of health in rural communities across the United States.

This toolkit will supplement and expand on previous work in this area including the RHIhub Social Determinants of Health for Rural People topic guide, and several RHIhub evidence-based toolkits for rural community health. This toolkit has seven different modules which includes:

  • Introduction
  • Program Models
  • Program Clearinghouse
  • Implementation
  • Evaluation
  • Sustainability
  • Dissemination

This toolkit is a step-by-step guide to help you build effective community health. Resources and examples are drawn from evidence-based and promising programs. By learning from programs that are known to be effective, you can make the best use of limited funding and resources.
