The Role of Social Determinants in Promoting Health and Health Equity

An issue brief from the Henry J. Kaiser Family Foundation notes that health initiatives increasingly seek integrated solutions that address social determinants of health such as socioeconomic status, education, the physical environment, employment, and social support networks. The brief, Beyond Health Care: The Role of Social Determinants in Promoting Health and Health Equity, highlights examples of mapping and place-based approaches that connect frequent users of emergency departments with primary care providers and social workers; a “health in all policies” approach that ensures decision makers across sectors are informed about the health, equity, and sustainability consequences of policy decisions; and Medicaid delivery and payment reform initiatives.
