NNEDLearn 2013 Participant: Asian Pacific Family Center
Description: The Health WISE program is an innovative community program that offers recovery-oriented, strength-based, and family-focused integrated care to Chinese immigrant consumers with mental illness and physical concerns and/ or substance abuse issues. A multidisciplinary team of counselors, primary care physicians, psychiatrists, case managers, health navigators and educators provide tailored and comprehensive services. Services are available in Mandarin and Cantonese and clients are not required to have health insurance.
Culturally competent health education and healthy lifestyle development are included as a part of the recovery process. Clients are provided free traditional Asian health services such as acupuncture and herbal medicine, when indicated.
Population of focus: Chinese immigrant consumers with mental illness and physical concerns and/ or substance abuse issues.

Background: A collaborative program between Asian Pacific Family Center, Pacific Clinics and Asian Pacific Healthcare Venture. The program began in July 2012 and is funded by the Los Angeles County Department of Mental Health.
